Qualifying for NHS Primary Care Trust Care Funding

This is funding for which you may qualify if your needs are of a particular type or of a sufficient level of severity. It is not means tested. If you qualify, 100% your funding is provided for by the NHS.

1. Are you or your family paying for your care at home or in a Nursing or Residential Home (because you have assets of more than £23,000)?

2. Have you got a nursing need as well as needing day to day assistance?

3. Have you been assessed by the Primary Care Trust for Continuing Care Funding (not the DSS means test)?

Yes

3a. If you answered Yes to Q3 above, has your physical and/or mental health deteriorated since the assessment was done?

Yes

3b. If you answered No to Q3 above, have you asked your GP to put together a team to do an assessment?

Yes

4. Have you been turned down for Continuing Care Funding?

5. Have you had Continuing Care Funding taken away after it was awarded?

Yes

6. Do you think you might have qualified for Continuing Care Funding for a while but did not know about it to claim it?
